First of all, I can't think of any great way to deal with multiple kids who play academy/select. You can allow multiple answers to a poll, but if you have the same answer for two kids, it's wrong again.
So if you have more than one kid, answer any way you want. Pick for your whole family, pick for just one of the kids... whatever you prefer.
Next, if your club was renamed, merged, acquired, or otherwise modified without you consciously saying "We are going to another club" then don't count that as a new club. If you or your child were playing at UFA and are on the GU DA team now, that's one club. That's true even if the merger or whatever caused you to go to a new location for your home fields. It still wasn't your conscious choice to move.
Really I'm just looking for the count of how many times you chose (for whatever reason) to pick up and move.

Just one club for 7 years now. We live less than 3 miles from the home fields so there would have to be some serious issues for us to make the change to another club.

Scattered all over - very sad actually - it was a great group of kids and parents but its also the age where some kids develop other interests or school gets harder. Still think the club could have tried harder to salvage the age group. We still miss our coach and group

I can't imagine taking your kid to another club to play soccer. Things must get pretty bad before I would commute for youth sports.

Lots of possible reasons... you might go to a club to join a higher level team not available at your club. You might leave to get on a specific great coach's team, or likewise you might leave to escape a very bad coach (coaching, ethics, etc.).
You might leave one club just to go to a closer one you didn't know about when you signed up. Or to go to one that didn't exist when you started.
You might follow a team, teammate, coach who leaves for their own reasons.
Lots of possible valid reasons!

Agreed jash. In addition, as in spectator's case, you might switch because there is no longer a team for your kid at your current club. This happens at smaller clubs.

Here's our story:
Started with rec at one club. Moved to another town and played with city rec league until we "aged out". Then moved to county rec league. Then switched to Academy at another club. After a couple of years in Academy, team relocated to another club at U13 and have been there since.
So looks like a total of 5 for us.
